LMM helps empower people to overcome barriers, obtain job skills, gain employment, locate safe and stable housing, access counseling and support services, stay out of prison, and secure second chances in community. Our mission is to promote shalom (peace, well-being) and justice (right relationships) through a Christian ministry of service and advocacy with those who are oppressed, forgotten and hurting.

Position Summary

The Clinical Coordinator is responsible for providing engagement, clinical services, and program support to Housing & Shelter clients. The Clinical Coordinator will serve as a clinical resource to assist clients providing crisis intervention, mental health case management, diagnostic assessment, and counseling services as appropriate.

Some of the duties include:

· Engage and provide linkage and/or clinical services to Housing & Shelter clients

· Ensure linkage and referral to Shelter staff partners and/or community providers as appropriate

· Assessment and diagnosis, treatment planning, case management, individual counseling, brief crisis counseling, treatment of mental and emotional disorders

· Complete crisis assessment and intervention for clients in psychiatric crisis

· Complete risk assessments, execute appropriate crisis treatment plan (i.e. hospital/probate)

· Meet with clients who have recently overdosed, interview and offer services for linkage to detox facilities

· Manage caseload and work in partnership with clinical team at the Shelter

· Ability to implement wrap-around case management

· Tracking and reporting of pertinent data elements including for grant reporting, progress notes in EHR, information entry into HOPE database

· Maintain productivity standards for clinical services, maintain quality standards with OMHAS and COA

· Manage resident conflicts and use de-escalation techniques from training (Handle with Care)

· Utilize Best Practices in Shelter services, including understanding and application of Housing First philosophy

· Apply Best Practices in Clinical services, including understanding and application of Trauma-Informed Care and other relevant interventions and philosophies

Qualifications

Our ideal candidate has Minimum of a Bachelor’s degree in social work, counseling or related field; LSW/LPC required. Some experience working with people experiencing homelessness, veterans, people with substance use disorders, people with re-entry backgrounds, and/or other relevant social service populations. Must possess strong communication skills and attention to detail. Must be CPR/First Aid certified within 90 days of employment.
